Email an electronic copy of the Perkins Application which includes both the 2025-26 Perkins ApplicationÌý(in Word format), and FS-10 (in Excel format) to theÌý¿ì²¥ÊÓƵ of CTEÌýand send the hard copy Perkins Application and FS-10 with original signatures to:
